New Patient Registration

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address. To register you we require proof of address, such as a recent tenancy agreement, bank statement, or utility bill, and one form of photographic ID.   

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

Registration takes three working days, after which you may book an appointment or attend the walk-in clinic. Please contact reception for further information. 

Rights and Responsibilities

We respect the rights of our patients in terms of race, gender, social class, age, religion, sexual orientation, disability or medical condition and would expect the same from our patients. The practice follows a strict policy of confidentially at all times all information is protected and shared and shared only with the patient concerned.
